Job summary

Abode seeks a Service Coordinator in Napa, CA to provide housing services to individuals who are homeless. The role includes counseling, referrals, and developing individualized housing plans aimed at permanent stability.

You will coordinate with landlords and partners to ensure services are delivered effectively, with a focus on increasing housing stability and access to related supports. Evening/weekend availability is required as needed.

Qualifications

• Experience with homeless populations and case management.
• Bachelor’s degree or equivalent in related field.
• 2 years of case management for homeless or low-income individuals.
• Valid California driver’s license with clean DMV record.
• Flexible schedule to work evenings and weekends.

Responsibilities

• Provide housing services to participants, including landlord engagement.
• Coordinate services with partners to ensure health and housing outcomes.
• Develop individualized housing plans and track progress.
• Document services timely using standard formats (BIRP).
• Attend program meetings and collaborate with internal and external teams.
