Pacific Health Group is seeking a field-based professional to support Medi-Cal members in the community, coordinating housing stability services and care management. The role emphasizes housing navigation, tenancy sustainment, and community partnerships across Sonoma and surrounding counties.

Responsibilities include field assessments, documentation in real time, and collaboration with housing authorities, shelters, and healthcare providers to ensure timely housing placements and ongoing support

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Strong communication, empathy and cultural competence.

Responsabilités

• Provide Housing Transition Navigation and tenancy sustainment services.
• Coordinate housing deposits and documentation with landlords and agencies.
• Develop individualized housing care plans and coordinate services.
• Conduct field outreach, home visits, and community assessments.
• Maintain timely, compliant documentation and chart notes.
• Collaborate with interdisciplinary teams to ensure seamless care.
