Qualifications

• 0-2 years of clinical experience with case management.
• Active, unrestricted State RN license or LCSW or APSW in good standing.

Responsibilities

• Manage health care and coordinate care for clients to achieve optimal outcomes.
• Develop and implement an integrated care plan with member, caregivers, and providers.
• Monitor progress, document care plan in system, and adjust as needed.
• Conduct home visits or in-person assessments as required, and collaborate with ICT meetings.
