Job summary

Talentify is seeking a Healthcare - Case Manager I for a remote role serving the Cleveland, Toledo, and Ravenna areas. The position requires RN, LSW, or LISW licensure with a schedule Mon–Fri 8-5.

This 3+ month contract involves coordinating care plans, combining in-home visits with remote management to optimize clinical, financial, and quality-of-life outcomes. Responsibilities include assessing needs, developing care plans, coordinating with providers, and monitoring progress.

Qualifications

• Must hold active, unrestricted RN, LSW, or LISW licensure.
• Bachelor's degree in Nursing or MSW required.
• 0-2 years of clinical case management experience.
• Driver's license with good driving record.
• Ability to communicate and coordinate across multidisciplinary teams.

Responsibilities

• Provide case management services to members with chronic or complex conditions.
• Develop and implement a care plan in collaboration with member, caregiver, and providers.
• Assess member needs by collecting information from member, family/caregiver, hospital staff, physicians, and other providers.
• Monitor and adjust the care plan to achieve optimal outcomes.
