Job summary

Qualifications

• Bachelor’s degree in nursing or related field or Master’s in Social Work.
• Minimum 3 years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Strong customer service and interpersonal skills; analytical ability.

Responsibilities

• Conduct comprehensive assessment of biomedical, psychological, social and functional needs.
• Develop personalized patient-centered care plans to optimize care.
• Engage patients and families as part of the care team with advocacy and education.
• Coordinate care transitions and collaborate with care team for complex cases.
