Job summary

Hines & Associates, Inc. is seeking a Registered Nurse Case Manager to develop patient care plans and advocate for patients within a supportive team.

This full-time role is in-office initially with a path to a hybrid arrangement after orientation, offering flexibility between home and the office. Ideal candidates will have at least 2 years of case management-related experience and excellent communication skills, enabling effective collaboration with patients and healthcare providers.

Qualifications

• RN licensed in the state or with reciprocation in the office state.
• Minimum 2 years in case management orientation and/or relevant healthcare setting.

Responsibilities

• Develop and implement patient care plans to meet health care needs.
• Advocate for patients and coordinate services across care settings.
• Communicate effectively with patients, families, and health professionals.
