Job summary

Optima Medical Management Group is seeking a Community Health Worker (CHW) to serve as a trusted liaison between community members, healthcare providers, and social service organizations. You will work as a hybrid member of the pod Care Team, conducting outreach, delivering health education, and connecting members to medical and community resources to improve outcomes and reduce barriers to care.

The CHW will collaborate with Lead Care Managers, MAs, and RNs, supporting navigation of healthcare

Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management or member care.
• Valid California driver's license and vehicle insurance.

Responsibilities

• Conduct outreach and provide health education to community members.
• Assist individuals in accessing healthcare, social services, and resources.
• Assess social determinants of health and identify barriers to care.
• Support care coordination with providers and care teams.
• Document interactions and referrals in designated systems.
• Maintain confidentiality and HIPAA compliance.
• Travel for outreach visits using personal vehicle.
• Participate in community outreach events and health fairs.
