Job summary

Optima Medical Management Group is seeking a Community Health Worker to serve as a trusted liaison among community members, healthcare providers, and social services. This largely remote role involves outreach, health education, and connecting members to medical and community resources.

The CHW will work with a multidisciplinary Care Team, support care coordination, and help reduce barriers to access. Strong communication and cultural sensitivity are essential; California driver’s license is

Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management, member care, customer service, call center, or member care.
• Valid California driver’s license and vehicle insurance required.
• Must be able to travel using personal vehicle and mileage reimbursement applies.

Responsibilities

• Conduct in-person visits to educate on health topics and support member care.
• Assist individuals in accessing healthcare, social services, and community resources.
• Assess social determinants of health and identify barriers to care.
• Support care coordination with clinicians, care managers, social workers, and community agencies.
• Document interactions and referrals in designated systems; maintain confidentiality.
