Job summary

Optima Medical Management Group is seeking a Community Health Worker to serve as a trusted liaison among community members, healthcare providers, and social services. The CHW will work mostly remotely as part of a dispersed care-team and conduct outreach, education, and resource referral to improve health outcomes.

The role emphasizes addressing social determinants of health, coordinating with care managers, and helping members access medical and behavioral health services.

Qualifications

• High school diploma or GED required.
• Minimum of 1 year experience in case management, member care, customer service, call center, or member care.
• Community Health Worker (CHW) certification required.
• Valid California driver’s license and vehicle insurance required.

Responsibilities

• Serve as primary team member for in-person visits and health education.
• Assist individuals in accessing healthcare, social services, and community resources.
• Assess social determinants of health and identify barriers to care.
• Support care coordination with providers, care managers, social workers, and agencies.
• Work as mostly remote member of the pod Care Team with Lead Care Managers, MAs and RNs.
• Accompany members to appointments and assist with enrollment navigation.
• Maintain contact with members via calls, visits, outreach, and telehealth.
• Document interactions, assessments and referrals in designated systems.
• Advocate for members and resolve barriers to care.
• Participate in outreach events, health fairs, and education workshops.
• Maintain confidentiality and comply with regulations.
• Attend regular member huddles and report updates.
• Report in-person to the office as needed.
• Perform additional duties as assigned.
