Jaan Health is seeking a dynamic Certified Medical Assistant to join our Indianapolis care team as a Clinical Care Coordinator. You will support population health initiatives, perform high-volume patient outreach, and document care activities in major EHR systems.

The role bridges clinical care with operations, reporting to Clinical Operations. The position is initially hybrid with a transition to fully on-site; the team emphasizes patient-centered, team-based care and training across care

Qualifications

• Certified Medical Assistant (CMA) required.
• Minimum 3 years of care management experience in a primary care setting or similar.
• Experience with population health is preferred.
• Excellent communication and technology skills.
• Prior use of electronic health records and health IT systems.

Responsabilités

• Conduct high-volume patient outreach via phone, text, and digital platforms.
• Document patient interactions and care management activities in EHRs (EPIC, Cerner, Allscripts, eCW, etc).
• Support care management activities such as intake coordination, appointment scheduling, medication refills, and follow-ups.
• Assist with patient onboarding and enrollment processes; partner with client operations teams to ensure seamless service delivery.
• Deliver training sessions on care protocols, systems, and operations to care teams.
• Adapt to evolving workflows, technology, and program demands.
• Maintain compliant documentation and provide courteous, efficient patient interactions.
