Jaan Health is seeking dynamic Certified Medical Assistants (CMA) Clinical Care Coordinators to join our care team. You will report to the Clinical Operations Manager and support patient-centered, team-based care using a population management informatics tool.

The role emphasizes high-volume outreach by calls, texts, and care platform messaging, with documentation and workflow alignment across the care team.

Formación

• CMA required.
• Minimum of 3 years experience in care management at a primary care setting or similar.
• Excellent communication and technology skills.
• Prior use of electronic health records and other health care information systems required.
• Significant experience within a primary care setting (chronic care preferred).
• Proven problem-solver, capable of high-volume outreach and multi-tasking.
• Must have reliable internet and a private work environment.

Responsabilidades

• High-volume patient outreach via phone, text, and digital platforms.
• Document patient interactions in EHRs (EPIC, Cerner, Allscripts, eCW, etc).
• Support clinicians with intake, scheduling, medication refills, and follow-ups.
• Assist with patient onboarding and enrollment processes with client operations teams.
• Deliver training sessions on care protocols and systems to care teams.
• Adapt to evolving workflows and program demands.
• Ensure courteous, efficient routing and referrals to clinical staff.
