Placements24 in East London (Eastern Cape) is seeking a highly organized Case Manager to support individuals and families facing complex social challenges. This hybrid role mixes fieldwork in the East London community with remote planning tasks.

You will assess needs, develop intervention plans, connect clients to resources, advocate for rights, and document progress, with a focus on ethical practice and confidentiality.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• 2–3 years of case management or social services experience.
• Knowledge of welfare systems, resources, and relevant laws.
• Strong assessment, planning, and problem-solving skills.
• Excellent interpersonal, communication, and listening abilities.
• Ability to work independently in a hybrid role.
• Proficiency with case management software and MS Office.
• Valid driver's license and vehicle for community work.

Responsibilities

• Assess client needs and develop individualized case plans with goals and timelines.
• Link clients to community resources and coordinate services.
• Provide ongoing support, monitoring, and follow-up.
• Advocate for clients' rights and access to services.
• Maintain confidential client records and notes.
• Collaborate with professionals and agencies for coordinated care.
• Respond to crises and provide referrals as needed.
• Prepare reports for funding bodies and internal reviews.
• Monitor program effectiveness and contribute to improvements.
