Job summary

IPMG in Hendricks County, Indiana is seeking a full-time Case Manager for a hybrid role combining remote work with regular in-person community meetings. You must reside in Hendricks County and be able to travel within the county and surrounding areas for meetings.

You will educate individuals about the Medicaid waiver process, develop individualized support plans based on strengths and goals, assist with accessing services, and coordinate regular team meetings with providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field + 1 year direct experience with people with intellectual/developmental disabilities

Two years experience with aging population
