Job summary

You will educate individuals about Medicaid waiver processes, create individualized support plans, help access services, and coordinate with care providers.

Qualifications

• Bachelor’s degree in a human services field or related credential.
• Valid driver's license, car insurance, and reliable transportation.
• Able to pass thorough background screens.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education about Medicaid waiver processes.
• Create an Individual support plan based on client strengths, needs, goals, and desires.
• Help individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings with providers as needed.
