Job summary

IPMG is hiring a Case Manager to work in a full-time hybrid role serving Wayne County, Indiana. The position combines remote work from home with in-person meetings and travel up to about an hour within the county for community meetings.

Responsibilities include educating individuals about Medicaid waivers, creating personalized support plans, helping access needed services, and coordinating with care teams. Applicants must reside in Wayne County and be able to travel as required.

Qualifications

• Bachelor's degree in a human services field or related experience.
• Ability to travel within Wayne County and surrounding counties.
• Must have valid driver’s license and reliable transportation.
• Ability to pass background checks.

Responsibilities

• Provide information and education to help individuals understand Medicaid waivers.
• Create an individual support plan based on individuals' strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings and collaboration with providers.
