Job summary

IPMG is seeking a Case Manager for a full-time hybrid role serving Tippecanoe County, Indiana. The position blends remote work from a home office with travel for community meetings several times a week.

You will educate individuals on Medicaid waivers, develop individualized support plans, and coordinate services to help clients achieve their goals, with travel within the county and surrounding areas as needed.

Qualifications

• Bachelor's degree in a human services field or related degree with relevant experience.
• Experience with Medicaid waivers or community-based services is preferred.
• Strong understanding of person-centered planning and goal setting.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens and work in a home-based hybrid setting.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on strengths, needs, goals, and desires.
• Help individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans and collaborate with providers.
