Job summary

IPMG is seeking a Case Manager to support individuals in Hendricks County, Indiana. This full-time hybrid role blends remote work with in-person community meetings several times a week and requires residing within Hendricks County.

You will assess needs, develop individual support plans, help access services, and coordinate with a team to review progress. A Bachelor's degree and valid driver's license are required.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
