Indiana Professional Management Group in Indiana is seeking a Case Manager for a hybrid role serving Harrison County. The position combines remote work with travel to attend community-based meetings several times a week; applicants must reside in Harrison County and be able to travel throughout the county.

The candidate should hold a Bachelor’s degree in a human services field (or related) or have equivalent experience, plus a valid driver’s license and pass background screenings.

Qualifications

• Bachelor’s degree in a human services field or related field, or a related combination with experience
• Valid driver’s license with reliable transportation
• Ability to travel within Harrison County and surrounding counties for meetings
• Must reside in Harrison County, Indiana and pass background screens
• Dedicated home office setup with computer and internet

Responsibilities

• Provide information and education about the Medicaid waiver process
• Create an Individual support plan based on individuals' strengths, needs, goals, and desires
• Help individuals access services and supports to meet goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with providers
