Job summary

Indiana Professional Management Group in Indiana is seeking a Case Manager for a full-time hybrid role. You will work from a home office with travel up to about an hour for community meetings, serving Owen County and surrounding areas.

You’ll educate individuals on Medicaid waivers, draft individual support plans, and coordinate services with a team of providers. A relevant bachelor’s degree and valid driver’s license are required.

Qualifications

• Bachelor’s degree in a human services field or related experience.
• Valid driver’s license, car insurance and reliable transportation.
• Ability to pass background screens.
• designated home office with computer and internet.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an individual support plan based on strengths, needs, goals, and desires of individuals served.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers.
