Job summary

Indiana Professional Management Group, Inc is hiring a Case Manager for a full-time hybrid role serving Tippecanoe County. You will work remotely from your home office with travel up to an hour for community-based meetings several times a week and attend in-person meetings as required within the county.

Responsibilities include educating individuals about Medicaid waivers, crafting individualized support plans, helping access services, and leading team meetings with providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience with people with intellectual and/or developmental disabilities, or two years experience with aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
