Job summary

Indiana Professional Management Group seeks a dedicated Case Manager to serve Owen County residents, combining remote work with local travel for in-person meetings. Applicants must reside in Owen County and be able to travel within the county and surrounding areas.

The role requires a human services bachelor’s degree or equivalent experience, a valid driver’s license, and the ability to coordinate Medicaid waiver services, develop individualized plans, and lead community team meetings.

Qualifications

• Bachelor’s degree in a qualifying human services field or relevant experience in the aging or disability sector.
• Valid driver's license and reliable transportation are required.
• Ability to pass thorough background screens and work from a designated home office.
• Willingness to travel within Owen County and surrounding counties as needed.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
