Job summary

Indiana Professional Management Group in Indiana is seeking a Case Manager for a full-time hybrid role serving Harrison County. The job involves remote work from home with travel for community meetings several times a week.

You will educate individuals about Medicaid waivers, develop individual support plans, connect services, and lead community-based team meetings. Applicants must reside in Harrison County, have a relevant degree or experience, reliable transportation, and pass background

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field with at least 1 year of direct experience with individuals with intellectual/developmental disabilities or 2 years with aging population.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers.
