Job summary

Indiana Professional Management Group in Indiana is hiring a full-time Case Manager for a hybrid role, combining remote work with travel to Marion County for meetings. Applicants must reside in Marion County for in-person sessions and travel throughout the county as needed.

You will educate individuals about Medicaid waivers, develop individualized support plans, help access services, and lead community-based team meetings with providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field plus 1 year of direct experience with people with intellectual/developmental disabilities or 2 years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens and establish a home office setup.

Responsibilities

• Provide information and education about Medicaid waiver processes.
• Create an individual support plan based on strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plans with service providers.
