Job summary

Indiana Professional Management Group is seeking a full-time Case Manager to work in a hybrid role serving Madison County, Indiana. The position requires residency within Madison County and travel to attend community-based meetings several times a week.

You will educate individuals about the Medicaid waiver process, develop individual support plans based on strengths, needs, and goals, and help access essential services. Collaboration with a range of providers is essential for client outcomes.

Qualifications

• Bachelor's degree in a human services field
• Valid driver's license, car insurance, and reliable transportation
• Ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smartphone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field
