Job summary

Indiana Professional Management Group, Inc is seeking a Case Manager to join our hybrid team serving Madison County, Indiana. The role blends remote work from a home office with local travel for community-based meetings several times weekly.

You will help individuals navigate the Medicaid waiver process, develop individualized support plans, coordinate services, and lead team meetings with providers to review plans of care.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or related human services field or a related Bachelor's degree with experience.
• Or a Bachelor's degree in an unrelated field with at least 1 year of direct experience with people with IDD, or 2 years with aging population.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual support plan based on individuals' strengths, needs, goals, and desires.
• Assist individuals in accessing services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans of care and collaborate with providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field; or a Bachelor's degree in an unrelated field with 1 year direct experience working with people with intellectual/developmental disabilities, or 2 years experience working with the aging population

Valid driver's license, car insurance, and reliable transportation
