Job summary

Indiana Professional Management Group, Inc (IPMG) is hiring a full-time Case Manager for a hybrid role combining remote work from home with travel to community meetings in Marion County, Indiana. Applicants must reside within Marion County and be willing to travel across the county for meetings several times a week.

You will inform clients about Medicaid waivers, craft individual support plans, connect them with needed services, and lead team meetings with providers to support goals and outcomes.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field
• OR a related bachelor’s with 1 year direct experience with individuals with ID/DD, or 2 years with aging population
• Valid driver’s license, car insurance, and reliable transportation

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
