Job summary

Indiana Professional Management Group is hiring for a Case Manager to work a full-time hybrid schedule with home-based tasks and in-person meetings within Grant County, Indiana. The role requires coordinating Medicaid waiver supports, building individual service plans, and connecting clients with needed services while attending community-based meetings.

The position emphasizes collaboration with teams, compliance with background checks, and reliable transportation for county travel.

Qualifications

• Bachelor's degree in a human services field or equivalent experience.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field and 1 year of direct experience with people with intellectual/developmental disabilities or 2 years with aging population
