Job summary

Indiana Professional Management Group, Inc is seeking a Case Manager for a full-time hybrid role serving Madison County, Indiana. The position combines remote work from a home office with travel within the county for community-based meetings several times a week.

Responsibilities include educating individuals on the Medicaid waiver process, creating tailored support plans, helping access needed services, and leading team meetings with providers to review care plans.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or a Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population

Responsibilities

• Provide information and education about the Medicaid waiver process
• Create an individual support plan based on the strengths, needs, goals, and desires of the individuals we serve
• Help individuals access services and supports to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
