Job summary

Indiana Professional Management Group (IPMG) seeks a full-time Case Manager for Madison County, Indiana. This hybrid role combines remote work from a home office with travel to attend community-based meetings several times a week.

You will develop Individual Support Plans, educate Individuals about Medicaid waivers, and coordinate with providers to meet goals within Madison County and nearby areas.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field with 1+ year direct experience with individuals having intellectual/developmental disabilities or 2+ years with aging population.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual Support Plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care with service providers.

Education
