Job summary

IPMG in Lafayette, IN, is hiring a full-time Case Manager. This hybrid role combines remote work from your home with local travel for community meetings across Tippecanoe County and surrounding areas. Candidates must reside in Tippecanoe County and be able to travel for in-person meetings.

Your role is to educate individuals on Medicaid waiver processes, create tailored support plans, help access services, and facilitate team meetings with providers to review progress and goals.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience with people with intellectual and/or developmental disabilities, or two years experience with aging population.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
