Job summary

IPMG is seeking a Case Manager to work in a hybrid role serving Hamilton County, Indiana. The position requires residency within Hamilton County and occasional travel for community meetings several times per week.

You will educate individuals about the Medicaid waiver process, develop individualized support plans, help access needed services, and coordinate with a team to implement plans.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field plus 1 year direct experience with individuals with disabilities, or 2 years experience with aging population.
• Must have a valid driver's license, car insurance, and reliable transportation.
• Must pass thorough background screens.
• Designated home office with computer, internet and smartphone.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual Support Plan based on individuals' strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review care plans and coordinate with service providers.
