Job summary

IPMG is a full-time, hybrid Case Manager position serving Montgomery County, Indiana. You will educate individuals on Medicaid waivers, develop tailored support plans, and help them access needed services while coordinating with local providers.

The role requires residing in Montgomery County, Indiana, with travel to community-based meetings across the county. A bachelor's in a human services field is required or equivalent experience, plus a valid driver's license and home-office setup.

Qualifications

• Bachelor's degree in a human services field or related field as listed, or
• Bachelor's degree in an unrelated field plus relevant experience with disability or aging population
• Valid driver's license, car insurance, and reliable transportation
• Ability to pass background screenings
• Designated home office with computer, internet and smartphone

Responsibilities

• Provide information to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on strengths, needs, goals and desires
• Help Individuals access services and supports to meet goals
• Facilitate regular community-based team meetings to review plans and coordinate with providers

Education

Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation

Bachelor’s degree in an unrelated field and a minimum of one year of direct experience with individuals with disabilities or two years with aging population
