IPMG is looking for a full-time Case Manager to serve Vigo County, Indiana, with a hybrid work arrangement that combines remote home office time and travel for community-based meetings up to approximately one hour away. Applicants must reside within Vigo County and be able to travel for in-person meetings several times a week.

Qualificações

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field with at least one year of direct experience with individuals who have intellectual/developmental disabilities or two years with aging populations.

Responsabilidades

• Provide information and education to help individuals understand the Medicaid waiver process
• Create an Individual support plan based on the individuals' strengths, needs, goals, and desires
• Help individuals access services and supports to meet their goals
• Facilitate regular community-based team meetings to review plans and coordinate with service providers
