Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager for a full-time, hybrid role serving Henry County, Indiana. The role requires residence within Henry County and travel to community meetings within the county and surrounding areas, with some remote work from a home office.

You will coordinate Medicaid waiver services, develop person-centered plans, and connect individuals to needed supports.

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related field.
• Experience with individuals with intellectual/developmental disabilities or aging population.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass background screens and maintain a home office setup.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, or related human services field

Bachelor's degree in an unrelated field with 1+ year experience with IDD or 2+ years aging population
