Job summary

Developmental Pathways (DP) is seeking Active Case Managers to join our Case Management Team. You will deliver compassionate, person-centered service and coordinate supports for individuals receiving Medicaid Waiver and State General Fund services, conducting in-home assessments and coordinating service plans.

This hybrid role involves travel across Arapahoe, Douglas and Elbert counties, frequent in-person visits, and collaboration with families, providers, and internal teams.

Qualifications

• Bachelor’s Degree; or
• Five (5) years of experience in the field of Long-Term Services and Supports; or
• Some combination of education and relevant experience appropriate to the requirements of the position

Responsibilities

• Conduct in-home visits to complete assessments, monitor service provision, and facilitate service plan meetings
• Engage with families and providers to build trust and partnerships
• Collaborate with Aurora office and other departments to enhance service delivery
• Work from home including hosting virtual Zoom meetings and documenting services
• Drive between locations, travel within catchment area weekly to fulfill requirements
