Indiana Professional Management Group (IPMG) is seeking a Case Manager for a full-time hybrid position serving Lawrence County, Indiana. The role combines remote work with travel to community-based meetings throughout the county and surrounding areas as needed.

Applicants must reside in Lawrence County, Indiana and be able to travel for meetings. A Bachelor's degree in a human services field or related experience is required, along with a valid driver's license and ability to pass background

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

Responsabilités

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Formation

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field and 1 year direct experience with individuals with intellectual/developmental disabilities or 2 years aging population experience
