Job summary

Indiana Professional Management Group, Inc. seeks a full-time Case Manager to serve Hancock County, Indiana. This hybrid role combines remote work from home with occasional travel for community-based meetings, within the county and surrounding areas.

You'll educate individuals about Medicaid waivers, develop individualized plans, coordinate access to services, and lead team meetings with providers to achieve goals. A Bachelor's degree in a human services field or related experience is required.

Qualifications

• Bachelor's degree in a human services field or related experience as an alternative
• Valid driver's license, car insurance, and reliable transportation
• Ability to pass thorough background screens
• Designated home office with computer, internet, and smartphone

Responsibilities

• Provide information and education about Medicaid waiver process
• Create an Individual support plan based on strengths, needs, goals, and desires of Individuals served
• Help Individuals access services and supports to meet their goals
• Facilitate regular community-based team meetings to review plan of care with service providers
