Job summary

York County Government seeks a Case Management Specialist to deliver a full range of case management services in the drug and alcohol field in south-central Pennsylvania. You will assess client needs, determine level of care, coordinate access to treatment, and develop a comprehensive service plan to support the client's treatment episode.

Qualifications

• Bachelor's degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field with related experience.
• Or bachelor's degree with 18 college credits in sociology, social welfare, psychology or related social sciences and one year direct service experience in alcohol or other drug treatment setting.

Responsibilities

• Master case management activities: screening, assessment, placement, case coordination, level of care placement approval, continued stay/concurrent review, and documentation.
• Advocate for clients with other agencies or organizations to maximize benefits.
• Coordinate ancillary services (health care, transportation, education, vocational training, housing) to support treatment.
• Ensure continuity of treatment and appropriate referral and follow-up to promote recovery.
• Document client activities and maintain confidentiality per state/federal laws.
