Job summary

Commonwealth Of Pennsylvania seeks a Drug and Alcohol Case Management Specialist Trainee to guide clients through assessments, coordinate services, and update Recovery Plans. You will help individuals navigate recovery and access resources within Cambria County.

Responsibilities include client screening, service assessment, care determination, plan coordination, and documentation while ensuring confidentiality.

Qualifications

• Bachelor's degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or related field; or Bachelor's with 18 credits in related social sciences.
• Any equivalent combination of experience and training.
• Applicants may be within 3 months of graduating with a qualifying degree.

Responsibilities

• Screen clients by telephone and in person during initial referral.
• Assess service needs in office or community settings.
• Determine appropriate level of care based on placement guidelines.
• Coordinate access to services and update Recovery Plans as needed.
• Request funding authorization and complete required documentation.
• Document case activities and ensure confidentiality compliance.
