PMHCC, Inc. is seeking a BE Substance Abuse Specialist in Philadelphia, PA. The role focuses on case management for individuals with severe behavioral health needs, coordinating care across medical, psychiatric, and community resources.

The position requires CADC certification, a valid driver’s license, and flexibility for after-hours and weekend work. Applicants should have at least 5 years’ experience in community-based treatment settings, strong collaboration skills, and a commitment to

Qualifications

• High school diploma with at least 5 years’ experience working at CTT with an active CADC certification.
• Expertise in behavioral health with one or more of the following required: criminal justice system, substance abuse, vocational, housing or entitlement issues.
• Must have a valid driver’s license, auto insurance and access to a vehicle; able to work evenings and weekends and have knowledge of City transit system.

Responsibilities

• Assess individuals’ strengths, needs and wants and develop service plans with goals.
• Coordinate with multidisciplinary teams to ensure integrated care and continuity of services.
• Complete Environmental Matrix to determine level of care and deliver services accordingly.
• Monitor progress with monthly meetings and update plans per regulatory standards.
• Provide transportation to appointments and assist with accessing community resources.
• Participate in daily team meetings and clinical care activities to address complex cases.
• Complete required documentation in a timely manner.
• Provide after-hours coverage, including on-call rotations and evening/weekend shifts.
