UnitedHealth Group in Fishkill, NY is seeking a Care Team Associate to support Care Management processes across CCM, DM, TCM, ED Follow-Up and Social Services. The role is 40 hours/week, with a hybrid schedule (onsite plus three days from home) at 600 Westage Dr, Fishkill, NY 12524.

You will handle documentation, inquiries, census and data entry, and coordinate with CM managers and staff while delivering excellent customer service and maintaining PHI confidentiality.

Formación

• High School Diploma / GED is required.
• 2+ years of administrative support experience.
• 2+ years of medical terminology knowledge.
• 1+ years of Microsoft Office proficiency.
• 1+ years of ability to use multiple IT systems.
• Live within a commutable distance to Fishkill, NY.
• Access to a quiet home workspace with PHI security.
• Reliable high-speed internet at home.

Responsabilidades

• Document member interactions per documentation requirements.
• Respond to inquiries via phone, CM platform, portal, email, or RightFax.
• Monitor CM queues for CCM, DM, TCM, ED Follow-Up and SS referrals.
• Process CM requests and prepare administrative support for clinical staff.
• Make inbound/outbound calls for scheduling, surveys, and materials distribution.
• Prepare Census data for CM referrals and notify leaders of outstanding cases.
• Enter data into software applications accurately.
• Provide clerical support for projects and reporting needs.
• Deliver excellent customer service to internal and external customers.
• Attend required meetings and participate in ad hoc committees.
• Stay informed on health plan benefits and NCQA regulations.
• Identify opportunities to improve processes in member care.
• Maintain confidentiality and adhere to policies.
• Perform other related duties as assigned.
