Qualifications

• Current, unrestricted RN license.
• 2+ years of clinical experience.
• 1+ years of MS Office (Word, Excel, Outlook).
• Reside in Mecklenburg, NC or surrounding areas; remote work eligible.

Responsibilities

• Assess, plan and implement individualized care management interventions.
• Identify referrals to healthcare and community services.
• Develop and implement care plans across continuum as single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for members and families with the care team.
