The role combines remote work with community visits across Dutchess and Ulster Counties, requiring strong assessment, planning, and documentation in an electronic health record system. Bilingual English/Spanish is preferred, with a focus on person-centered care and improving health outcomes.

Qualifications

• Bachelor’s degree in psychology, Social Work, Nursing, Human Services, or related field; 2 years case management experience.
• Associate degree in related field with 4 years case management experience.
• Working knowledge of behavioral health terminology and client needs.
• Experience with Medicaid/Medicare and other insurances preferred.

Responsibilities

• Conduct comprehensive assessments of client needs including medical, mental health, housing, and social supports.
• Coordinate with healthcare providers to ensure seamless care and services.
• Develop and update person-centered care plans with client and family.
• Educate patients and families about conditions, medications, and self-management.
• Connect clients with community supports such as food banks, transportation, and legal aid.
• Monitor progress and document interventions in the EHR.
• Maintain timely documentation of activities and progress.
• Other duties as assigned.
