Job summary

Optum in Fishkill, NY is seeking a Care Team Associate to support complex case management, disease management, transitional care, ED follow-up, and social services. The role involves daily census management, coordinating with CM staff, and ensuring NCQA and CMS compliance while delivering outstanding service.

Schedule is 40 hours, Monday–Friday, with a possibility of three remote days. Candidates should meet education and experience requirements and reside within commuting distance to Fishkill,

Qualifications

• High School Diploma / GED required.
• 2+ years of administrative support experience.
• 2+ years of medical terminology knowledge.
• 1+ years of computer proficiency in Microsoft Office.
• 1+ years of ability to toggle between multiple IT systems and platforms.
• Live within a commutable distance to Fishkill, NY.
• Home workspace with PHI security and reliable internet.

Responsibilities

• Document member interactions concisely for CM/NCQA/CMS compliance.
• Respond to inquiries via phone, portal, email, or RightFax.
• Monitor CM queues for CCM/DM/TCM/ED Follow-Up/SS referrals.
• Process CM requests and prepare admin support for clinical staff.
• Make inbound/outbound calls for scheduling and surveys.
• Prepare census data for CM referrals and notify leaders of cases.
• Enter member contact information into software applications.
• Support clinical staff and managers on special projects.
• Provide excellent customer service to internal and external customers.
• Attend required meetings and participate in ad hoc committees.
