Job summary

CareVitality Inc. is hiring a Care Coordinator & Onboarding Specialist in Cypress, TX to support patients through consent for CCM, PCM and RPM.

You will train patients on home-use medical devices and help them follow their care plans, coordinating with providers and care teams. As onboarding progresses, you’ll conduct regular follow-ups, provide educational materials, and identify gaps in preventive services while documenting activities in CareVitality’s platform.

Qualifications

• Ability to collect patient consent for CCM, PCM, RPM and related programs.
• Experience documenting time spent with patients and managing caseloads.
• Proficiency with EHR, care management platforms, and standard Office apps.

Responsibilities

• Gather patient consent for care management and remote monitoring programs and train on home-use devices.
• Review and update allergy and medication profiles; support self-management of medications.
