Job summary

CareVitality Inc. is seeking a Care Coordinator & Onboarding Specialist to support patients by obtaining consent for CCM, PCM and RPM programs. You will train patients to use devices at home and work toward coordinating care plans.

The role evolves to regular outreach, gap analysis in care, and documentation in CareVitality's platform, with ongoing collaboration across providers and care teams.

Qualifications

• Ability to gather patient consent for care management and remote monitoring programs.
• Set up and train patients to use medical devices at home.
• Review and update allergy and medication profiles; support self-management of medications.

Responsibilities

• Initially focus on consent gathering and device training for home monitoring.
• As onboarding progresses, conduct monthly care plan reviews and coordinate with providers.
• Document activities in CareVitality platform; communicate gaps and coordinate with teams.
