Job summary

CareVitality Inc. is seeking a Care Coordinator & Onboarding Specialist in Houston to support patients with consent for CCM, PCM and RPM, train them on using home medical devices, and help coordinate care across providers and community resources.

Role evolves to monthly care-plan reviews, patient outreach, and documentation in CareVitality platform with collaboration across care teams. Office-based with potential future hybrid work.

Qualifications

• Gathers patient consent for CCM, PCM and RPM programs.
• Documents time spent with patients in EHR and tracks caseloads.
• Works with providers and teams to coordinate care.

Responsibilities

• Initially obtain patient consent for CCM, RPM and related programs.
• Train patients to use home medical devices and provide setup support.
• Conduct monthly follow-ups and update care plans.
• Coordinate services with providers and community resources.
• Document care activities in CareVitality platform and EHR as needed.
• Escalate complex cases to the Care Manager when appropriate.
