Job summary

Epiphany Family Services is seeking a TCM Care Manager to perform outreach, enrollment, and coordination within a hybrid care model across in-office, field, and remote settings. You will engage members, arrange services, and support the care team in connecting community resources.

Ideal candidates include those with lived behavioral health experience and a Certified Peer Support Specialist credential, plus at least one year of related paid work.

Qualifications

• Must have a high school diploma or equivalent.
• Certified Peer Support Specialist with lived experience preferred.
• 1 year of paid experience performing extender functions or related duties.

Responsibilities

• Outreach, engagement, and follow-up with members.
• Coordinate services/appointments and arrange transportation.
• Enroll members into Tailored Care Management services.
• Promote health and share knowledge with the care team.
• Provide and track referrals to community resources.
