Qualifications

• Bachelor’s degree required; Master’s preferred.
• Experience in care coordination or case management.
• Experience with behavioral health or social barriers is a plus.

Responsibilities

• Conduct comprehensive assessments of physical, behavioral health, and social needs.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across medical, behavioral health, housing, and social services.
• Provide ongoing follow-up and health promotion.
