Job summary

Epiphany Family Services seeks a TCM Care Manager to engage members, coordinate services, and support enrollment in tailored care management. The role involves outreach, scheduling, and collaboration with a care team across in-office, remote, and field settings.

The ideal candidate has lived behavioral health experience, a Certified Peer Support Specialist credential, and at least 1 year of related paid work. Hybrid work model with travel within the service area is required.

Qualifications

• Must be 18 years or older
• Have a high school diploma or equivalent
• Be a person with lived behavioral health experience and a Certified Peer Support Specialist
• Have one year of paid frontline experience as described in Extender Functions
• Possess NC CHW SCCT or related credentials
• Ability to meet qualifications including CHW or Navigator background

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments and transportation reminders.
• Conduct introductory engagement calls to enroll members in TCM services.
• Engage in health promotion and knowledge sharing with the team.
• Share member information with care manager and team.
• Provide and track referrals; assist in obtaining community resources and social support services.
• Support addressing unmet health-related resource needs and urgent TCM needs.
• Other duties as assigned.
